AMENDMENT NUMBER FOUR
TO AMBULANCE SERVICES AGREEMENT
This Amendment is made and entered into this 21" day of July, 2015 ("Effective Date")
by and between the CITY OF COSTA MESA, a mumicipal corporation ("City") and CARE
AMBULANCE SERVICE, INC., a California corporation ("Contractor").
WHEREAS, City and Contractor entered into an agreement on September 1, 2010, for
Contractor to provide Basic Life Support (BLS), Advanced Life Support (ALS) emergency
ambulance services, and patient billing services in the City (the "Agreement") for an initial
three-year term; and
WHEREAS, the parties subsequently entered into amendments extending the term
pursuant to Section 5.2 of the Agreement, which allows for extensions providing that the total
Agreement lasts no more than ten years; and
WHEREAS, the parties now desire to enter into this Amendment to further extend the
Agreement term as permitted by Section 5.2 of the Agreement.
NOW, THEREFORE, the parties agree as follows:
1. The term of the Agreement set forth in Section 5.1 is hereby extended, and such
extension shall commence on September 17, 2015, at 7:30 a.m. and end on June 30, 2016, at
7:30 a.m.
2. All other terms, conditions, and provisions of the Agreement not in conflict with
this Amendment shall remain in full force and effect.
IN WITNESS WHEREOF, the parties hereto have set their hand by their duly authorized
