Contract Acceptance and Award Confirmation
The Lodi Unified School District (Lodi USD) having co -solicited the Request for Proposal #010615 for Office, School,
and other Workplace -Related Supplies and Services, has awarded a contract to the following vendor:
Staples Contract & Commercial, Inc. operating as Staples Business Advantage
1. The Contract Acceptance and Award between NJPA and Staples Business Advantage was executed by NJPA on
January 20, 2015, and by Staples Business Advantage on February 11, 2015. The effective date of the four-year
contract is August 1, 2015, which is the first day after the previous contract expired.
2. The Lodi USD's Board of Education approved this contract award during its regular meeting on February 17,
2015, as part of Consent Agenda A, Item A-6, "Resolution 2015-6—Award of Request for Proposal (RFP) for
Office, School and Other Workplace -related Supplies and Services Co -solicited with National Joint Powers
Alliance (NJPA)." (See page three of the Minutes of the Regular Meeting of the Board of Education.)
3. The sole purpose of this Contract Acceptance and Award Confirmation is to more formally verify Lodi USD's
approval of this contract (Contract Number 010615 -SCC) between Lodi USD and Staples Business Advantage.
