From the above stated findings, we recommend the removal and replacement of the ENTRY, LIVING ROOM, KITCHEN, DINING ROOM, NOOK and about half of the DEN. This removal may be limited to slab areas 12—inches away from interior and exterior walls as follows : 1. Score the concrete slab 11/2" deep about 12—inches away from interior and exterior walls (so as not to cut the existing welded—wire mesh) . 2. Cut the slab 12" away from the score line. 3. Break and remove the center of the slab . 4. Remove the sand and the visqueen membrane. 5. Remove the saturated soil and replace with sand (2 to 4 inches thick) 6. Replace the 6—mil visqueen overlaid with 1—inch of sand. 7. Install # 3 bars at 18" o. c. each way at the center depth of a 4" slab . 8. Pour a new concrete slab (f'c = 2000. psi) 9. Allow the new slab to *cure' for 28—days prior to installing the new tile floor at the ENTRY.
