Dear Building Official:
Southwest Inspection and Testing, Inc. was retained by the owner to provide special inspection and
materials testing in the areas that were required by section1701 of the Uniform Building Code,
approved projects drawings and specifications, this was witnessed by the following special inspectors.
Concrete placement and pre-check was observed and compressive samples were cast by, Bruce Nielsen
(OCEMA# 298). Compressive strength samples were picked up by Southwest and tested in our
laboratory'. All work was done in accordance with the project plans, specifications and workmanship
provisions of the Uniform Building Code.
Stressing of'A" post tensioned tendons was inspected and elongations were recorded by , Brad Kaahanui
(ICBO# 1032736-89). Accordance with the projects plans, specifications and workmanship provisions
of the Uniform Building Code.
All work was done in conformance with the Structural Engineers recommendations, approved drawings
and/or revisions, project specifications and applicable workmanship provisions of the Uniform Building
