HomeMy WebLinkAbout- - Attachment II - 1/5/2010ATTACHMENT II AMENDMENT NUMBER TWO TO AGREEMENT FOR WORKER'S COMPENSATION CLAIMS ADMINISTRATION This Amendment is made and entered into this 1"5 th day of December, 2009, ("Effective Date") by and between the City of Costa Mesa, a municipal corporation ("City") and CorVel Corporation ("Consultant"). WHEREAS, Consultant and City entered into an agreement on April 18, 2006, for dedicated worker's compensation claims administration for the City; and WHEREAS, the term of Consultant and City's original Agreement expired on August 31, 2008 and was subsequently extended until December 31, 2009; and WHEREAS, Consultant and City now wish to amend the Agreement to extend the term of the Agreement to allow Consultant to continue providing services to City until a Request for Proposal is prepared, the bidding process has been completed and a new Agreement with the selected company has been executed. NOW, THEREFORE, for valuable consideration, the receipt and sufficiency of which is hereby acknowledged, the Parties agree as follows: Paragraph 4.1 shall be amended to read, "This Agreement shall commence on April 18, 2006 and continue until January 5, 2010, unless previously terminated as provided herein or as otherwise agreed to in writing by the parties. 2. All terms not herein defined shall have the same meaning and use as set forth in the Agreement. 3. All other terms, conditions, and provisions of the Agreement not in conflict with this Amendment, shall remain in full force and effect.
