RECOMMENDATION:
Authorize the sole source purchase of a cardiac resuscitation system, consisting of
eight (8) monitor -defibrillators and associated accessories, from Zoll Medical
Corporation 269 Mill Road, Chelmsford, MA 01824. The total net purchase cost,
including state and local tax shall be $229,129.60.
BACKGROUND:
In order to provide Paramedic / Advanced Life Support -level care for the residents of
Costa Mesa, the Fire Department is required to carry manual cardiac monitor -
defibrillators on all of its paramedic apparatus. These units perform critical diagnostic
and treatment interventions necessary to provide the appropriate care for patients with
cardiac and other life-threatening emergencies.
Because the cardiac monitor -defibrillators currently in use by the Fire Department have
become obsolete due to improved technology, and also because they have exceeded
their 5 -year industry -standard field deployment, it is thus necessary to purchase new
units. Of the three companies that sell such units (Zoll, Physio Control, Phillips), only
the Zoll Corporation has the cardiac resuscitation system that meets the needs and
specifications of the Fire Department. The Zoll cardiac resuscitation system consists of
the Zoll "E -Series" monitor -defibrillator and the Zoll "AutoPulse" automatic CPR chest
compression device, which are used in conjunction with each other in the treatment of
cardiac arrest victims.
It is important to note that American Heart Association Emergency Cardiovascular Care
scientific guidelines emphasize the criticality of high quality CPR chest compressions to
maximize survival from cardiac arrest. High quality CPR chest compressions depend
not only on proper depth and rate, but also minimal interruptions during compressions to
maintain adequate cerebral and coronary perfusion pressures. Studies clearly
demonstrate that the Zoll system achieves the highest quality CPR.

1. Equipment Integration & Compatibility — Only the Zoll system is designed for,
and capable of, electronic interface between the Zoll "E -Series" monitor -defibrillator
and the Zoll "AutoPulse" automatic CPR chest compression device.
2. Superior Chest Compression Technology — The Zoll "AutoPulse" device
performs the highest quality CPR chest compressions in comparison to the other
automatic CPR device on the market in terms of blood flow and perfusion.
3. Exclusivity in Approval for Use — The Zoll "AutoPulse" is the only automatic CPR
chest compression device that is approved for use in Orange County by the Orange
County Emergency Medical Services Agency.
4. Superior Capnography Technology — Only the Zoll "E -Series" monitor -defibrillator
enables paramedics to utilize both sidestream and mainstream technology to
perform capnography to assess the level of exhaled carbon dioxide.
5. Inter -Agency Operability — The Zoll "E -Series" monitor -defibrillator is becoming the
predominant unit amongst Orange County fire agencies. For example, surrounding
agencies such a Newport Beach Fire Department, Santa Ana Fire Department, and
Orange County Fire Authority (the largest fire agency in the county), all use Zoll
monitor -defibrillators. It is very helpful on mutual aid incidents for agencies to be
familiar with each other's equipment.
6. Improved Technology: Real -Time CPR Quality Analysis — The Zoll "E -Series"
monitor -defibrillator has a feature that provides real-time feedback to firefighters
when performing manual CPR that helps ensure the compressions and
ventilations administered are of the correct depth, rate, and timing.
7. Improved Technology: "See -Through CPR" — The Zoll "E -Series" monitor -
defibrillator has a feature that allows paramedics to determine the patient's
cardiac rhythm without interrupting chest compressions. The American Heart
Association has determined that delivering consistent high quality CPR with
minimal interruptions significantly improves the chance of survival for cardiac
arrest victims.
ALTERNATIVES CONSIDERED:
This current proposal represents the alternative. The original proposal was for 9 "E -
Series" monitor -defibrillators and 8 "AutoPulse" devices. Due to budget constraints,
however, the Fire Department has modified the original request to 8 and 3 units,
respectively, which would be the absolute minimum needed. (Rather than put AutoPulse
units on all apparatus, the modified plan is to put one AutoPulse unit on each of the 3
Care ambulances that are dedicated to our City. Also, we would have one less
monitor -defibrillator for mutual aid situations).

FISCAL REVIEW:
Funding in the amount of $229,140 for the purchase of a cardiac resuscitation system has
been appropriated in the fiscal year 2010-11 budget, approved by City Council on June 22,
2010.
LEGAL REVIEW:
None required.
CONCLUSION:
For all of the aforementioned reasons, the Fire Department respectfully requests that
the City Council approve the issuance of a sole source purchase order to Zoll Medical
Corporation for the purchase of the Zoll Medical Corporation's cardiac resuscitation
system, consisting of eight (8) "E -Series" monitor -defibrillators and three (3) "AutoPulse"
automatic CPR chest compression devices and associated accessories, for a total cost
of $229,129.60.
