Proposed Service: Employment services program will provide income -earning
opportunities for adults with developmental disabilities.
Amount Requested: $27,000 for salary costs of salesperson that secures paid work
for the work services program and program staff.

Proiect Independence — Employment For All Program
Proposed Service: Employee training program to assist enhance skills in order to
develop jobs for developmentally disabled individuals.

Citv of Costa Mesa — HCD Homeless Outreach Proeram
Proposed Service: Direct engagement of chronic CM homeless; carry out needs
assessment and provide linkages to appropriate housing and
support services.
Amount Requested: $35,000 for contract program staff.

Colette's Children's Home — Transitional Housing Program
Proposed Service: Program will provide housing and supportive services for
homeless women and children.

Serving PeoDle in Need (SPIN) — Substance Abuse Recover Program
Proposed Program: Program will focus on providing housing and substance
addiction recovery services for chronic CM homeless.
Amount Requested: $40,000 for program and admin staff salaries. Funds will also
be used for housing placement services.

Alzheimer's Familv Services Center —Adult Davcare Program
Proposed Service: Program will provide adult daycare services for persons with
Alzheimer's and other forms of dementia. Support services
for caregivers are also provided.

Communitv Senior Serve - Congregate Meals
Proposed Service: The program provides seniors with hot lunch 5 days per week
at the Costa Mesa Senior Center.
