You are hereby directed to make the herein described changes or do the following described work not included in the original scope of
work approved with the original contract or previously approved change orders.
NOTE: This change order is not effective until approved in accordance with Council Policy No. 700-1.
Description of work be done, estimate of quantities, and prices to be paid. Segregate between additional work at contract price,
agreed price and force account. Unless otherwise stated, rates for rental of equipment cover only such time as equipment is actually
used and no allowance will be made for idle time.

EXTRA WORK AT NEGOTIATED AND CALCULATED TIME AND MATERIALS PRICES (Negotiated prices include all
markups allowed by contract):
Item 1 — Landscape and Electrical Modifications:
The City authorizes the Contractor to implement landscape and electrical improvements in the parkways and medians in
accordance with the revised plans. The change order price is based on unit costs of various items of work as agreed to by
the City and may be revised based on actual quantities. The change order also provides the contractor with 15 additional
days to complete the project and includes all mark up and overhead.
Negotiated Change Order Price: $358,000.00
Add: 15.0 working days.

Item 2 — Quantity Adjustments:
Payment for increased quantities of structural section and other removals over the estimated quantities during design. This
payment is based on extension of unit prices provided during bid and no further compensation is allowed. No additional
time is granted for this item.

hereby agree, if this proposal is approved, that we will provide all equipment, furnish all
materials, except as may otherwise be noted above, and perform all services necessary for
the work above specified, and will accept as full payment therefor the prices shown above.
